Morphologic Change of the Kidney resulting from Disseminated Intravascular Coagulation(DIC) in the rat



Abstract
Morphologic study of the kidney in DIC was largely limited to acute massive lesions induced by various pregnancy-associated complications and severe infection of pathogenic microorganisms. To examine the renal lesions in subacute or chronic DIC state, an experimental model was studied. Fibrin-thrombotic lesions were produced by transfusion of incompatible blood(3cc) via tail vein of the Wistar rat. The morphologic changes were observed light and electron microscopically from 12 hours to 14 days after transfusion.
The results were as follows.
1. The glomerular lesions are composed of multiple fibrin thrombi in the capillary loop, irregular thickening of the basement membrane, the endothelial cell swelling with mild proliferations and obliteration of the capillary lumen.
These light microscopic features were characteristically focal and segmental lesions.
2. Multiple thrombi are also shown in the interlobar artery, arteriole, venule and small vein in the interstitium.
Focal degeneration and necrosis of the proximal tubule is evident and more severe in the perivascular area.
3. The earliest lesion in electron microscopic observation is fibrin thrombi composed of the degenerated platelets, red blood cells and fibrinoid material.
Destruction of the endothelial lining, large mass of fibrin, fibrinoid deposition in the subendothelial region and basement membrane, irregular thickening with splitting of the basement membrane, degenerative changes of the endothelial cell and many phagolysosome with vesicles in the mesangial cells are observed during the experimental period. But the foot process of the endothelial cell is relatively intact.
Above focal glomerular and tubular lesions are relatively same features of DIC induced by other causes, and possibly produced by occlusive fibrin thrombi in microcirculation following transfusion of incompatible blood.
